Private Pay services are self pay services. Upon admission, a service agreement detailing billing rates and payment requirements is signed by the client or their authorized representative. Clients are billed monthly for the services they receive.

Services available through Private Pay include:

· Personal care assistance,

· Homemaking,

· Respite care,

· Emergency response systems,

· Medical transportation,

· Nursing,

· Physical therapy,

· Occupational therapy,

· Speech therapy, and

· Social worker.

The PASSPORT Medicaid Waiver program is administered by the Ohio Department of Aging and overseen by the Area Agency on Aging. The PASSPORT program provides home and community based services to eligible older Ohio residents. To qualify for PASSPORT you must be:

· Age 60 or better;

· Need hands-on assistance with dressing, bathing, toileting, grooming, eating or mobility;

· Financially eligible for Medicaid institutional care;

· Frail enough to require a nursing home level of care; and

· Able to remain safely at home with the consent of their physician.
